The San Diego North County Home Visiting Collaborative is a dedicated group of home visiting providers and supporters who partner together to serve North County children and families. The Collaborative began in 2014 to increase knowledge of home visiting programs and services in North County; identify gaps in services and strategies to address the gaps; to share tools, resources, and service delivery best practices; to strengthen effective referral pathways for those seeking services; and to promote strong communication and coordination between home visiting providers and supporters. Individuals and families are linked to the resources, services and programs that best fit their needs. Overall, the San Diego North County Home Visiting Collaborative strengthens and streamlines the service delivery system, and raises awareness of the short-term and long-term value of home visiting services and support.
